What matters isn't how tall it looks on the bed. It's what remains under the weight of your head — and whether that height fits your body and how you sleep.

The pillow is a fitting problem.

A meta-analysis pooling nine high-quality trials and more than 550 participants found that neck alignment while side-sleeping tracked shape and height, not what the pillow was filled with. A separate 2025 review of pillow studies reached a similarly modest, materials-agnostic conclusion. Across the literature, filling carries very little of the effect — height and shape relative to shoulder width carry nearly all of it.

And it keeps changing overnight.

A pillow that fits when you first lie down can flatten well before morning. Down and low-resilience foams behave very differently under sustained load — which means the support condition your neck experiences at 3 a.m. may not be the one you fell asleep on.

Fit follows position — and shoulder width.

Side

Needs the most height

The pillow is filling the distance between the mattress and the side of the head — a distance set by shoulder width. That's why one height rarely serves every side sleeper.

Back

Needs moderate height

Enough to support the curve of the neck — without pushing the chin toward the chest.

Front

Needs almost none

Any real loft forces the neck into rotation and extension at once — one reason this position is so poorly served by a standard pillow.

These are fitting principles drawn from the research literature, not a medical prescription. Anatomy, comfort and how you actually sleep through the night all vary — and most people shift position more than once before morning.
